Community Health Workers (CHW) are frontline public health workers who are trusted community members. They have a close understanding of the communities they serve.
A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. CHWs are connections to resources in the community and play a vital role in advancing health equity.
CHWs refer community members to health and social systems and assist with navigation of systems and organizations. They are a vital voice in marginalized communities and advocate for those at risk.
